  2. Payment: The $50 fee can be paid via cash, card, or check.
  3. Receiving Documentation: Once processed, your FMLA documentation will be sent to you either by email or mail.

  1. Pre-Surgery Consultation: Discuss with your surgeon your expected recovery time to understand how long you might need to be away from work.
  2. FMLA Documentation: Obtain the necessary paperwork from our office, noting the $50 processing fee.
